It’s best practice to place TENS unit pads (electrodes) near the painful/injured area. This type of placement is called contiguous placement. Part of effectively using a TENS unit for relieving muscle pain involves understanding where to place the electrodes. Generally speaking, TENS electrodes should be placed around the perimeter of the area to be treated in order to be effective. This allows the current to travel through the nerve fibers within the affected muscles.

Whenever using a TENS Machine, two pads are placed over or at the perimeter of the injured area of the body. The unit sends an electrical current, or an electrical pulse, from and to the pads whenever activated. This electrical pulse stimulates the release of the body’s own natural opioid pain-relieving chemicals.
TENS Unit Pad Placement for Common Pain Sites Upper Back Pain: For pain on one side of your upper back, you can place a pad right on the pain (not on the spine, but on soft tissue) and one at least an inch away. You might want to move the second pad around to determine where you find the most relief.

Researchers have not been able to conclusively establish that using the TENS unit can address a person’s erectile dysfunction. Many researchers feel that electrodes should not be brought anywhere near the genitals. That said, the TENS unit is said to be effective in addressing problems of pelvic floor muscles.
Here you can see a common example of the correct electrode placement with the back electrodes 33 x 4 cm. For this application you need a TENS unit with at least two channels. Place the large back electrodes parallel to each other on your back as shown in the picture. These special electrodes are designed so that they can usually be placed alone.
